LifeSmart is a case management program available to employers in the area. The program educates, motivates and guides individual employees to achieve and maintain their best possible health and effectively manage the healthcare costs of their employer's health plan. Through lifestyle intervention and education, many people can better manage their illness and make changes in their daily habits that can delay or even prevent disease.

At LifeSmart, the client enters into a cooperative relationship with a caring registered nurse. Together, they explore the client's personal health risks and alternatives to enhance health. They also work collaboratively to set goals to improve self-care management and promote wellness.

Meeting at the workplace, at home or through telephone conversations, each employee's needs are reassessed, progress is monitored and goals are re-evaluated to keep motivation high to make healthy changes.
