I certify that the information given by me in this employment application is accurate and complete to the best of my knowledge and understand that, if employed, falsified statements or material omissions from this application shall be grounds for dismissal. I understand that nothing contained in this employment application, or in any oral representations made to me during the interview process, may create an employment contract between Stanly Regional Medical Center and myself. Furthermore, I understand and agree that, if hired, my employment is for no definite period and that just as I am free to resign at any time, Stanly Regional Medical Center reserves the right to terminate my employment at any time, with or without cause and without prior notice, regardless of the date of payment of my wages and salary.
2. I authorize Stanly Regional Medical Center to investigate the information in this application, to contact my previous employers and to secure additional job-related information about me. I hereby release Stanly Regional Medical Center from liability relating to its investigation and I release all other persons, corporations, or organizations furnishing information to Stanly Regional Medical Center.
3. All offers of employment to candidates are contingent based on the ability to pass a drug screening and completion of a full background check. In the event that the applicant fails to pass the drug screening or Stanly Regional Medical Center receives disqualifying information as a result of the background check, Stanly Regional Medical Center reserves the right to rescind the offer of employment and terminate employment immediately.
4. Tobacco use is not permitted within the facilities or on the grounds of Stanly Regional Medical Center or at any offices, programs or facilities staffed offsite by the hospital. Tobacco use is not permitted in privately owned vehicles located on hospital property, in any hospital vehicle or in vehicles in use for hospital business.
