Since 1995, our medical center has offered outpatient programming specifically designed for patients dealing with a chronic illness like diabetes, heart disease or congestive heart failure. Led by skilled health care professionals in disease management, these programs have a proven track record of positive outcomes for their patients.

Did you know that more than 20 million men, women and children in America live with diabetes every day? If you’re one of them, these special programs can help you manage the disease and reduce its impact on your life.
Nationally recognized by the American Diabetes Association (ADA), this program offers an eight-week session plus one follow-up class. Led by an interdisciplinary team consisting of certified diabetes educators, registered dietitians and registered nurses, the program covers the 10 content areas outlined by the ADA. A wellness store is available for the purchase of products recommended in the program.

This is a service offered to diabetes program participants that exhibit “foot triggers” (numbness, tingling, pain or burning in feet). These patients receive a thorough foot or leg assessment, debridement of toenails and education regarding prevention of foot ulcers. An interdisciplinary team approach is used that involves registered nurses and a family nurse practitioner. Call (704) 984-4189 for more information.
Stanly Regional’s Diabetes Support Group hosts monthly educational get-togethers on diabetes and meets the third Tuesday of each month at 6 p.m.

This three-hour program reviews the basics of controlling diabetes, including new information on meal planning, nutrition tips, exercise and an inspirational strategy on how to stay positive with diabetes. This program is available to prior participants of the eight-week Diabetes Self-Management Program.

Evidence-based medicine shows diabetes can be prevented! Attend our five-session program instructed by a team of certified diabetes educators including Registered Nurses and Registered Dietitians to learn and practice prevention strategies. Participants at high risk for developing diabetes are encouraged to attend and will receive workbook, blood sugar meter, temporary membership to YMCA and more.

Stanly Regional Medical Center offers a Gestational Diabetes Program to keep you and your baby healthy during this very special time. Your health care team includes a registered dietitian and a registered nurse who will teach important skills, including meal-planning guidelines, monitoring blood sugars, exercise techniques and medication options.

Our certified Pump Trainers will assist in the evaluation of the insulin pump as an alternative for insulin delivery. Pump instruction, fine-tuning blood sugars and nutrition therapy are provided on-site.

Working closely with program participants and their personal physicians, our staff helps to develop individual treatment plans that support optimal recovery and rehabilitation.
The rehabilitation staff consists of physicians, registered nurses, exercise specialists, a registered dietitian, psychologist and vocational rehabilitation counselor.
The Cardiac Rehabilitation Program at Stanly Regional Medical Center is fully certified by the North Carolina Department of Health and Human Services. Working with the Stanly County Family YMCA, participants benefit from the resources of two organizations that focus on health and wellness.

LifeSmart is a case management program available to employers in the area. The program educates, motivates and guides individual employees to achieve and maintain their best possible health, and effectively manage the health care costs of their employer's health plan. Through lifestyle intervention and education, many people can better manage their illness and make changes in their daily habits that can delay or even prevent disease.
At LifeSmart, the client enters into a cooperative relationship with a caring registered nurse. Together, they explore the client's personal health risks and alternatives to enhance health. They also work collaboratively to set goals to improve self-care management and promote wellness.
Meeting at the workplace, at home or through telephone conversations, each employee's needs are reassessed, progress monitored and goals re-evaluated to keep motivation high to make healthy changes.
The nurse provides education to the client and family about medications, early signs and symptoms of complications, disease management and wellness promotion. The LifeSmart nurse helps the employee identify barriers and set goals specific to needs.
